If your TriCare plan requires a referral for a specialist, please have your PCM fax your referral to (385) 430-0710.
If you are traveling from outside of Utah, be sure to confirm with your plan that you have out-of-state benefits. Not all plans have coverage outside of the state in which you reside for “non-emergency” services. Telehealth availability depends on state-by-state licensing laws. Our team can help determine if you are telehealth eligible.

We are an outpatient facility open during normal business hours M-F from 8am-5pm.

Appointments must be cancelled at least 48 hours prior to the scheduled appointment time. Procedures must be cancelled at least 72 hours prior to the scheduled procedure. There is a nonrefundable fee of $100 for no-shows and cancellations that fall outside of the required window.

Yes! We do accept some insurance plans and are working to add additional carriers. We will continue to update the list above as those options grow.

It depends on the test. Some labs can take up to 12 weeks for results to be returned. This is especially common with specialty labs and genetic testing. Once your provider has a chance to review them, our care team may contact you to schedule an appointment to discuss abnormal lab results and answer your in-depth questions.
